About the Opportunity

Springbrook Wellness is expanding services at this Article 28 primary care clinic in Oneonta, NY, designed to improve access to care for individuals with developmental disabilities. We are seeking a Family Nurse Practitioner who is passionate about patient-centered care and excited by the opportunity to make a meaningful impact in a new clinic setting.

This is a great opportunity for an FNP who wants to do more than see patients. You will have the chance to help shape early clinic workflows, support a specialized patient population, and be part of a mission-driven organization addressing an important need in the community.

We will consider Full time or Part time hours. Sign on Bonus amount will prorated based on days per week worked.

What You’ll Do

  • Provide primary care services to patients, with a focus on individuals with developmental disabilities
  • Complete patient assessments, diagnosis, treatment planning, follow-up care, and health education
  • Collaborate with clinical leadership, physicians, nursing staff, care teams, and community partners
  • Support the development of clinic processes, workflows, and patient care standards
  • Communicate clearly and compassionately with patients, families, caregivers, and support teams
  • Help create a positive, respectful, and accessible care experience for every patient

Compensation & Benefits

The salary range for this role is $120,000–$160,000, depending on experience.

Sign On Bonus Details

Award of up to $10,000. Full time pay out will be paid as follows:

$2,500 after 90 days of successful employment
$2,500 after 6 months
$5,000 after 12 months
